چکیده

Cryoballoon ablation (CBA) has been widely accepted as a major technic for pulmonary vein isolation (PVI) in patients with atrial fibrillation (AF). PV anatomy is an important factor to obtain circumferential balloon adhesion that results occlusion achieve acute PVI. The KODEX-EPD (Philips, Amsterdam, Netherlands) dielectric-based electroanatomic mapping system can display panoramic (PANO) view of the left atrium and evaluate without other imaging modalities. Prior studies demonstrated high ovality potential mechanism impeding formation durable lesion. However, accuracy assessed by how it influences performance tool PVI are still unknown. To assess using system, them their impact on We enrolled 10 who underwent primary CBA paroxysmal AF all had pre-procedural cardiac CT or MRI. Ovality index (OI) was defined ratio maximum diameter divided minimum which measured under PANO CT/MRI images sagittal view. An evaluation obtained Occlusion Viewer intracardiac echography each CB-PV engagement. Single shot at first freezing attempt after complete PV. Fifty-eight balloon-PV engagements 39 PVs were analyzed. Consistent status between KODEX ICE 50 (86%) [complete occlusion: 30/34 (88%), incomplete 20/24 (83%)]. OI significantly larger than (2.0±0.5 vs 1.6 ±0.4. p=0.03). Representative cases shown Figure1A B. achieved 33 (84%). All showed KODEX, single smaller (1.6±0.4 2.1±0.4, p=0.003). A positive correlation found (Figure1C). novel feasible may facilitate CB AF. Higher associated lower rate.

Spastic occlusion of coronary artery during cryoballoon pulmonary vein isolation

Introduction Recently, pulmonary vein isolation (PVI) with cryoballoon (CB) has been widely accepted as treatment for atrial fibrillation (AF). Especially, second-generation CB (Arctic Front Advance, Medtronic, Inc, Minneapolis, MN) has a high cooling effect, which can achieve durable lesions of pulmonary vein (PV) antrum and better clinical outcomes. On the other hand, several complications, s...

Conjoined Inferior Pulmonary Veins during Pulmonary Vein Isolation: Prevalence and Novel Approach for Pulmonary Vein Isolation with Cryoballoon.

Cardiac resynchronization therapy is an established treatment modality in heart failure. Though non-response is a serious issue. To address this issue, a good understanding of the electrical activation during left bundle branch block, biventricular, as well as right- and left ventricular pacing is needed. This way by interpreting the 12-lead electrocardiogram, possible reasons for suboptimal tr...

Transesophageal echocardiography during pulmonary vein cryoballoon ablation for atrial fibrillation.

We describe our first 20 cases of cryoablation of atrial fibrillation (AF) using transesophageal echocardiography (TEE). Continuous procedural monitoring with TEE by a cardiologist and senior sonographer assists the electrophysiologist in performance of the cryoballoon procedure of AF. Previously using intracardiac echocardiography (ICE) we have found TEE to have better overall procedural imagi...

Breakage of a Circular Catheter Wedged in a Right Pulmonary Vein during Cryoballoon Pulmonary Vein Isolation

A 50-year-old man with paroxysmal atrial fibrillation underwent pulmonary vein isolation (PVI) using a cryoballoon. During the PVI procedure, a circular diagnostic catheter (CDC) was wedged deeply into the right PV (RPV). The lung tissues seemed to be tightening around the CDC. After various attempts to resolve the situation, the shaft of the CDC was completely torn, and the circular part remai...

Direct pressure monitoring accurately predicts pulmonary vein occlusion during cryoballoon ablation.

Cryoballoon ablation (CBA) is an established therapy for atrial fibrillation (AF). Pulmonary vein (PV) occlusion is essential for achieving antral contact and PV isolation and is typically assessed by contrast injection. We present a novel method of direct pressure monitoring for assessment of PV occlusion. Transcatheter pressure is monitored during balloon advancement to the PV antrum. Pressur...
